Back to Caring, carers and long-term conditions. If you're off work sick for 7 days or less, your employer should not ask for medical evidence that you've been ill. Instead they can ask you to confirm that you've been ill. You can do this by filling in a form yourself when you return to work. This is called self-certification. If you're off work sick for more than 7 days, your employer will usually ask for a fit note or Statement of Fitness for Work from a GP or hospital doctor.
Where the doctor says the employee might be fit for work, they can give details of what level of work they consider the person is able to do. For example, a factory employee with a back injury might be able to come back to work to do light tasks, but would not be able to do any heavy lifting. The doctor might suggest in the fit note ways the employer can help their employee get back to work.
For example:. The employer should carefully consider any fit note recommendations as they can help the person get back to work quicker.
There is no need to get another fit note. Some workplaces might have an occupational health scheme or employee assistance programme EAP to help the return to work process. If the employee has a disability, by law the employer must consider making reasonable adjustments to help them return to work and carry out their job.
For example to:. If the employee does not wish the employer to see their medical information, the employer will have to make a decision based on what they know and are told by the employee. Any information the employer does get about an employee must be stored and kept confidential.
Only appropriate senior and involved members of staff should have access to the information. Please book a routine appointment; urgent appointments are only provided for genuine medical emergencies and not for the purpose of certificates.
In either case there are rules governing the issue of sick certificates and the GP may not be able to supply one, depending on the information you provide. If you are under the care of a hospital, your certificate may be issued by the hospital, rather than by the practice. There is usually no charge for providing a fit note if you are off sick for more than seven days. Some employers may request a fit note e. This is a private non-NHS medical certificate.
If your certificate runs out, but you are still sick, you will need to consult the doctor again before you can get a further certificate. Fit certificates can be back-dated so it is not necessary to make an emergency appointment to renew your certifiate. If you attend an emergency appointment for the purpose of getting a medical certificate you will be asked to rebook.

If you need a fit note, contact your GP surgery. Or if you are getting hospital treatment, ask for one from your hospital doctor. If they deem that you are "not fit for work" they will write a note to that effect that you can present to your employer. They may think that you "may be fit for work taking into account the following advice", under which circumstances they would layout circumstances or changes to your usual working routine that would allow you to perform your job - or some variation of it.
If your employer is unable to accommodate these conditions, the note is considered to say "unfit for work".
